PDID Form (Personal Digital Identity)

Personal Digital Identity (PDID) Request Form

The request form is now entirely online! This form can be used for a New Employees, Name Changes, Updates or Deletions. When submitting for a New Employee an account will be created to access UNC computers, Ursa and email. A follow-up email will be sent to the new user and the department contact noted to see if additional access will be needed.

Note: If you are a student employee and are needing to do web time entry or need to access your pay stub you supervisor does not need to submit a PDID form.

Follow these steps to submit a PDID Form.

Step 1

Go to http://help.unco.edu and click on the "Submit a Ticket" link.



Step 2

Click on the pull down menu next to the Website Request Type and choose "PDID Form".



Step 3

Fill out the Request Fields. The "Contact Info" at the top will be the Supervisor's information. All "Requestor" fields are also the Supervisor's information. All "Users" fields will be for the new employee or the employee we're changing the information for.



Click the "Finish" button to submit the PDID.
